MY PATH's Basic Package is perfect for you if you need a little organization and motivation to take the next step along your future path. A MY PATH Employment Specialist will partner with you to develop your plan for the future.

Base Package Includes

  • Initial Consultation & Intake Process

  • Customized "MY PATH Action Plan"

  • Action Plan Revisions

  • 2 in-person meetings per month (1 hr each)

  • Email check-in and motivation

  • Communication with family

Base Package Add-Ons

  • 3 in-person meetings per month

  • 4 in-person meetings per month

  • 6 in-person meetings per month
